South Indian skin, which predominantly falls in the Fitzpatrick skin type III to V range, requires a careful, specifically adapted approach to melasma treatment. The same aggressive peels and lasers that work beautifully on lighter skin types can trigger severe post-inflammatory hyperpigmentation (PIH) — making the melasma far worse — in darker skin.
What Is Melasma — And How Is It Different from Other Pigmentation?
Melasma is a chronic, acquired hyperpigmentation disorder driven primarily by hormonal influence and UV exposure. It appears as symmetrical, irregular brown to grey-brown patches — most commonly on the cheeks, upper lip, nose bridge, and forehead.
Unlike sunspots or post-acne marks, melasma has a dermal component — meaning the pigment is deposited deeper in the skin, not just in the surface layers. This is why it is more resistant to treatment and more prone to relapse. A dermatoscope or Wood's lamp examination helps classify melasma depth, which guides treatment selection.

Why South Indian / Darker Skin Is More Vulnerable to Melasma
Melanocytes (the cells that produce pigment) in Fitzpatrick III–V skin are more active and reactive than in lighter skin types. They produce more melanin in response to UV, hormones, and inflammation. This is a double vulnerability:
- • The skin is more prone to developing melasma in the first place
- • Any treatment that causes inflammation or irritation can trigger PIH — darkening the very area you are trying to lighten
This is why “brightening” treatments used aggressively on lighter skin (strong TCA peels, ablative lasers, intense retinoid protocols) can cause disaster in South Indian skin without careful patient selection and preparation.
The Melasma Treatment Hierarchy: What Actually Works
Step 1: Sunscreen — Non-Negotiable, Daily, Lifelong
Sunscreen is not just a supportive measure — it is the treatment. UV radiation is the primary driver of melasma. Without daily SPF 50+ broad-spectrum (UVA + UVB) sunscreen, no treatment will produce lasting results. Patients who do not use sunscreen diligently will relapse within weeks of stopping treatment.
For South Indian skin: tinted sunscreens with iron oxide provide additional protection against visible light, which can also trigger melasma — particularly the dermal type. Physical/mineral sunscreens (zinc oxide, titanium dioxide) are preferred for those with sensitive or post-procedure skin.
Step 2: Topical Depigmenting Agents
First-line medical treatment for melasma is typically a prescribed topical combination, which may include:
- • Tranexamic acid — increasingly preferred in Indian skin for its safety profile and efficacy in melasma
- • Kojic acid — well-tolerated, effective for medium-depth pigmentation
- • Niacinamide — anti-inflammatory, inhibits melanin transfer to skin cells
- • Azelaic acid — gentle and effective, particularly suitable for sensitive skin
- • Hydroquinone (prescription-only) — gold standard depigmenting agent but must be used under medical supervision in cycles (not continuously)
These agents require consistent use over 3–6 months before results are visible. Patience and compliance are essential.
Step 3: Superficial Chemical Peels (With Pre-Treatment Protocol)
Superficial chemical peels — glycolic acid, lactic acid, kojic acid peels — can accelerate improvement in melasma in darker skin types, but only with proper preparation.
The pre-peel protocol (priming the skin with topical agents including sunscreen and often a depigmenting agent for 4–6 weeks beforehand) dramatically reduces the risk of PIH. Without this preparation, peels in South Indian skin carry a real risk of making the pigmentation worse.
Step 4: Advanced Treatments — For Resistant Melasma
- • Oral tranexamic acid — increasingly used in India for resistant melasma with good evidence
- • Microneedling with topicals — delivers depigmenting agents deeper into the skin
- • Low-fluence Q-switched Nd:YAG laser — the preferred laser for darker skin when laser is indicated; must be performed by experienced practitioners
What Does Not Work — And What Makes Melasma Worse
- • Fairness creams with hidden steroids — initial lightening followed by severe rebound and steroid-induced skin damage
- • Aggressive peels without preparation — high risk of PIH in South Indian skin
- • Ablative fractional lasers in untrained hands — can worsen melasma dramatically
- • Skipping sunscreen — undoes all treatment progress within days
- • Lemon juice, turmeric masks, home remedies — can cause chemical burns, allergic reactions, and PIH in susceptible skin
Managing Realistic Expectations
Melasma is a chronic condition — not a one-time problem with a one-time cure. With proper treatment, most patients achieve 60–80% improvement. However, melasma can and does recur — particularly with sun exposure, hormonal changes (pregnancy, hormonal contraception), and stress.
